Listed below are the permit application forms for individual non-storm water NPDES permits to operate a wastewater disposal system and/ or to discharge industrial or domestic wastewater. These forms should not be used to apply for general permits for storm water, onsite systems, mining and quarry operations, water well construction related wastewater, pesticide discharges, hydrostatic testing, tank ballasting, or water line discharges, dewatering activities, and residential geothermal discharges; or for permits for open or confined animal feeding operations. Please refer to the specific program webpages listed in the left-hand navigation bar to apply for a permit for any of these discharges.
The individual permit application forms were updated in May 2019. The new forms have a different look than the old forms. In addition, minor changes in the required information have been made to the industrial forms. You may have received some of the old forms from the department; we will continue to accept these through June 30, 2020. Please note, the Supplement – Sulfate and Chloride Data is not needed with the new versions of Form 2 and Form 3 for industrial facilities.
