NPDES Application Forms, Description

The forms shown on the NPDES Forms page are used to apply for permits to operate a wastewater disposal system and to discharge industrial or domestic wastewater. These forms should not be used to apply for general or individual storm water permits or permits for open or confined animal feeding operations. Please refer to the Storm water and Animal Feeding Operations web pages for more information.

Renewal applications are due 180 days before an existing NPDES permit expires. Applications for new dischargers are due at least 180 days prior to the date of first discharge. NPDES permits are placed on public notice for 30 days before being issued. If comments are received and a hearing is required, the Department would schedule a hearing and respond to comments received at the hearing, which may require an additional 60 to 90 days.

All entities that hold an industrial, non-industrial, or domestic NPDES permit are required to pay a facility-specific annual fee, and all a fee of $85 is required for all permit applications (renewal or new). Annual fees for General Permit #5 are required with the submission of the Notice of Intent for Coverage. Please refer to the NPDES Fees page for more information.

Industrial Wastewater
Forms 1, 2, 2F, 3, 4 and 5 and the Supplement for TDS and Chloride Data are applications for individual permits for industrial wastewater discharges. The file "General Information and Glossary of Terms" contains general information on the application process for industrial facilities and should be consulted to determine which forms are applicable to your discharge(s). Form 1, Form 5 and the Supplement for TDS Data must be submitted by all applicants together with one or more of the other four forms. Please use the file "Application Review Checklist" to make sure you have completed all necessary portions of the application forms. The file named "Useful Links" is designed to assist with the completion of the forms. Non-Industrial or Domestic Wastewater (For Municipal or Semi-public Facilities)

Form 30 must be used by publicly-owned and privately-owned (non-industrial) systems that treat primarily domestic sewage. The file "Application Overview" should be consulted to determine which forms are applicable to your discharge(s). Form 30 Part A and Part F must be submitted by all applicants. Please use the file "Application Review Checklist" to make sure you have completed all necessary portions of the application forms. The file named "Useful Links" is designed to assist with the completion of the forms. Form 31 is a treatment agreement and must be completed for each major contributing industry that discharges wastewater to another treatment system for final disposal.

Clean Water State Revolving Fund (CWSRF) loan funds can be used by municipalities and sanitary districts to finance the design and construction of almost all publicly owned wastewater treatment and conveyance improvements. State Revolving Fund Loan Program offers communities and sanitary districts low interest loans for the construction of wastewater treatment and collection system improvements. This loan program is available for modifying or constructing publicly-owned wastewater projects only. General Permit #5
The Notice of Intent for Coverage under General Permit #5 is used to apply for coverage under Iowa's general permit for wastewater discharges from mining and processing operations (quarry and sand and gravel).
