This form has been designed to assist applicants in ititiating the permit process with both the U.S. Corps of Engineers and the Iowa Department of Natural Resources agencies. This form is used for construction, excavation or filling in a water of the U.S. or on a floodplain. Instructions and application forms are available from the link provided above. Parts of this form also contain a description of the activities that are regulated by these agencies, the applicant's role in protecting Iowa's waters, an explanation of the agencies' roles, and a description of the application process.

Flooding

Construction, excavation or filling in rivers, streams, lakes, wetlands, or on flood plains may require permits from both agencies. This application form is to be used to begin the permit process for any of the following activities:

  • cutting the bank of a river or stream;
  • any excavation or dredging in a stream or river channel;
  • channel changes or relocation (including stream straightening);
  • construction of any permanent dock, pier, wharf, beach, intake, or outfall structure on a stream, river, or lake;
  • placement of any fill, riprap, or material in a stream, river channel, lake or wetland;
  • construction of a dam across any waterway;
  • placement of fill, construction of levees, roadways and bridges; and similar activities on a flood plain;
  • construction of buildings on a flood plain.

This application form is broken into four different parts. The first part is an introduction explaining the need for this application, the role of the agencies and the applicant, as well as how to report unauthorized construction. The second part focuses on instructions; it explains how to fill out the form. The third part is an example of an application form and drawing plans. While the forth part is the actual application form and a necessary sheet provided for drawing plans.
