Crossbow for Deer and Turkey Hunting License Application, Form 542-1369

A permit which allows the use of a crossbow while bowhunting for deer and/or turkey for persons who are certified by a physician as having a disability in the upper extremities.
Group Home Annual Fishing Permit Application (*.pdf, Form 542-8058)
A free annual permit for health care facilities and juvenile shelters which allows patients to fish as a supervised group without individual licenses.
Motor Vehicle Use Permit for Nonambulatory Persons Application, Form 542-8057

A permit which allows for the use of a motor vehicle on game management areas or state parks for persons who are certified by a physician as having a disability which makes it impossible to ambulate without the use of a motor vehicle.
Special Annual Resident Hunting and Fishing License Application (*.pdf, Form 542-8056)
A free annual hunting and fishing license for low-income persons who are 65 and older or for low-income persons who are permanently disabled.
Resident Severely Disabled Deer Hunting Application and Affidavit (*.pdf file)
Iowa residents who are severely disabled may obtain a paid deer hunting license to hunt during the Youth and Disabled Hunter Season.

A free lifetime fishing license for persons who are certified by a physician as having a permanent severe physical or mental disability.
