New Title V Application Forms Available
New Part 2 application forms for the Title V operating permit program have been developed in collaboration with stakeholders who participated in a 2012 process improvement event.
The new Part 2 forms, which identify all applicable requirements and plans for demonstrating ongoing compliance, allow for flexible submittal of information, are easier for applicants to use and result in a faster, more seamless permit review process. To get the most efficiency from the revised forms, applicants are encouraged to use the Microsoft Word version of the forms and to submit their applications through the State Permitting and Air Reporting System (SPARS).
To coordinate with the new Part 2 forms, the Part 1 form 1.3 and Part 3 forms were also updated. Both the new and the previous forms are available at http://www.iowadnr.gov/InsideDNR/RegulatoryAir/OperatingPermits.aspx. The DNR will accept both versions of the Part 2 forms through August 15, 2014. After August 15, 2014 only the new Part 2 application forms will be accepted.
