Title V Form Changes Go Into Effect April 1, 2008

Iowa DNR recently made changes to Title V application forms. Title V facilities must use the new forms to submit applications (e.g., Title V permit initial, renewal, or modification applications) after April 1, 2008. The new forms can be downloaded from the Air Quality Bureau’s website at www.iowacleanair.com/prof/oper/opapp.html.

Title V form changes do not involve forms for emission inventories and fee payments. Use the same Form 4.0 and Form 5.0 as you did to submit 2007 emission inventories and fee payments.

All necessary changes will also be made to the SPARS system. If you submit Title V applications using the SPARS Web after April 1, 2008, please follow the SPARS Web instructions.

The major changes are:

  1. Form 1.1 Plant Location & Layout Diagram is not needed anymore. This form was used to submit a plant layout diagram and to identify the Universal Transverse Mercator (UTM) coordinates of the plant UTM locator. It is not needed to submit a plant layout diagram. The requirement to submit a plant UTM locator is replaced by requirements to identify UTM coordinates for each individual emission point on modified Form 2.0 (see below).
  2. Modified Form 2.0 requires UTM coordinates of each emission point instead of the relative distance from the plant UTM locator. UTM zone (either 14 or 15) and North America Datum (NAD 27 or NAD 83) must be identified. The modified Form 2.0 also drops the requirements for building dimensions and stack exhaust velocity, emission rate, and moisture. Please see "Operating Permit Application Instructions" on how to complete this form.
  3. All Part 2 forms were modified, but most of those changes involve only updates to regulations, instructions and descriptions. However, Form RE-05 was changed to add CAIR and CAMR requirements, and Form RE-08 was changed to add a table for listing all units subject to CAM requirements.
