We want your volunteer experience to be safe, memorable and above all fun. Please bring the following forms with you to your volunteer event or submit them ahead of time.
Volunteer Waiver & Liability Form - Everyone volunteering at the DNR must complete this form. If a volunteer is under the age of 18, a parent or guardian must sign off on the form.
Background Check Form - Volunteers serving more than 8 hours a month or with vulnerable populations must complete a background check from ahead of time.
Authorization to Drive a State Vehicle Form - If you will be driving a state vehicle while volunteering, please complete the attached form.
